         Микрогемациркуляция в сердце, или циркуляция крови по коронарным микрососудам сердца, или кровообращение по коронарным микрососудам сердца - это разновидность локального кровотока, непрерывный поток крови (кровоток) по микрососудам (микрогемациркуляторное русло) сердца в соответствии с его потенциальными и актуальными потребностями.
         Эти потребности удовлетворяются посредством следующих частных транспортных функций кровообращения:
             доставка от системы пищеварения, системы дыхания к тканям сердца воды, минеральных веществ, питательных веществ, кислорода;
             удаление от тканей сердца к системам выделения конечных продуктов метаболизма;
              распределение тепла в организме;
             доставка и удаление от исполнительных звеньев регуляторов систем к их объектам управления гуморально активных веществ - средств управления структурами и функциями организма.
         В каждой ткани организма кровоток регулируется во-первых с целью обеспечения соответствия кровотока в ткани её метаболическим потребностям общим для любых тканей и, во-вторых, с целью обеспечения соответствия кровотока в ткани её специфическим функциям. Все ткани нуждаются в доставке питательных веществ (глюкоза, аминокислоты, жирные кислоты и т.д.), кислорода, в удалении конечных продуктов метаболизма. Поток крови в ткани предназначен для удовлетворения этих общих для всех тканей потребностей. Недостаток питательных веществ, недостаток кислорода, накопление конечных продуктов метаболизма в ткани вместе или в отдельности могут послужить непосредственной причиной увеличения кровотока в ткани. Кроме общих метаболических потребностей в разных тканях могут быть специфические потребности, детерминанты специфических функций ткани органа.
         Главной специфической функцией сердца является перекачивание крови по системе кровообращения, то есть нагнетание крови в артерии и обеспечение движения крови по кровеносным сосудам в соответствии с потенциальными и актуальными потребностями всех органов и тканей организма. Для эффективного осуществления этой специфической функции предназначено регулирование потока крови в коронарных сосудах сердца. Отсюда, изменения специфической функции тканей сердца и нарушение соответствия актуальных процессов потенциальным потребностям могут быть причиной изменения кровотока в этом органе и его тканях.
         Кровоснабжение тканей сердца обеспечивается по правой и левой венечным артериям. Это первые артерии, ответвляющиеся от начала восходящей части аорты. Собственные крупные вены сердца собираются в венечный синус, впадающий в правое предсердие. Туда же впадают и мелкие вены сердца. В сердце имеется густая капиллярная сеть.          Примечания.

         1. Микро-гема-циркуляция или микро-гем-о-циркуляция, гема-динамика или гем-о-динамика?
         Сложные слова в литературном русском языке могут образовываться соединением словообразовательных основ без использования соединительных гласных (микро-гема-циркуляция) или с использованием соединительных гласных «о» или «е» (гем-о-глобин). Сложное слово «микрогемациркуляция» содержит три основы: микро-гема-циркуляция. Его вторая основа «гема - кровь» полностью совпадает с корнем соответствующего слова, является неделимой частью его лексического значения. Сложное слово «гема-циркуляция» означает циркуляцию крови, кровообращение. Сложное слово « гемоглобин» содержит две основы: « гем» и «глобин», объединенных соединительной гласной «о». Основа «гем» (пигмент крови); имеет иное лексическое значение, чем основа «гема» (кровь);. Поэтому более определенным, правильным будет писать гема-циркуляция, микро-гема-циркуляция, но не гем-о-циркуляция. Аналогично, правильным было бы писать гема-динамика, но не гем-о-динамика, поскольку гемадинамика изучает динамику крови (гема), а не динамику пигмента (гем).
         2. Циркуляция» (circulation) и кровообращение (гемациркуляция, циркуляция крови) - не синонимы.
         Отдельно слово «циркуляция» (circulation) не следует использовать в качестве синонима терминов кровообращение, или гемациркуляция, или циркуляция крови. Очевидна смысловая разница между словом «циркуляция» (вообще, чего угодно: лимфы, солей жёлчных кислот, газов) и словосочетанием «циркуляция крови».
         Микрогемациркуляторное русло - это дистальная часть сердечно-сосудистой системы, расположенная между артериями и венами. В микрогемациркуляторном русле посредством микроциркуляции осуществляется взаимодействие крови с другими тканями. Здесь система кровообращения и система крови актуализируют достижение своего предназначения - завершают выполнение многообразных транспортных функций по взаимообмену частей организма, организма в целом и его среды веществами и энергией.

         Микрогемациркуляторное русло начинается самым мелким артериальным сосудом - артериолой. Артериолы продолжаются последовательно предкапиллярами, капиллярами, посткапиллярами, от которых происходят венулы. Предкапилляры нередко называют терминальными (конечными) артериолами, или метартериолами. В месте перехода от предкапилляров к капиллярам расположены предкапиллярные гладкомышечные сфинктеры. Сфинктеры - это один из исполнительных механизмов в управлении кровотоком в микрогемациркуляторном русле. Сеть капилляров представляет собой сложную динамическую структуру с различными меняющимися диаметрами сосудов. Некоторые из капилляров наиболее предпочтительны для кровотока. По сравнению с другими капиллярами они имеют наибольший диаметр. Путь движения крови по этим капиллярам называют предпочтительным каналом (преференциальным каналом, preferential channel). В месте отхождения обычных капилляров от капилляра, являющегося преференциальным каналом, также расположены гладкомышечные капиллярные сфинктеры.
         В микрогемациркуляторном русле имеется много артериоловенулярных анастомозов. Эти анастомозы также имеют управляемые гладкомышечные сфинктеры.
         В некоторых органах (почка, печень) микрогемациркуляторное русло построено иначе. Так, к клубочку почечного тельца подходит артерия (приносящий сосуд) и выходит из него также артерия (выносящий сосуд). Капиллярную сеть, вставленную между двумя однотипными сосудами (артериями), называют артериальной «чудесной сетью».
         По типу «чудесной сети» построена капиллярная сеть в дольке печени - венозная «чудесная сеть».

         Артериальная кровь доставляется к тканям сердца по правой и левой венечным артериям, а венозная кровь отводится по соответствующим венам. Собственные крупные вены сердца собираются в венечный синус, впадающий в правое предсердие. Туда же впадают и мелкие вены сердца. В сердце имеется густая капиллярная сеть. Каждое мышечное волокно сопровождается капиллярами. Главные правая и левая коронарные артерии, которые снабжают ткани сердца, являются первыми сосудами, отходящими от аорты. Отсюда, как и в других системных органах, движущей силой, определяющей кровоток в миокарде, является системное артериальное давление. Большая часть крови, протекающей через ткани миокарда, возвращается в правое предсердие через большую сердечную вену (коронарный синус). Регулирование кровотока в венечных сосудах. Локальная метаболическая регуляция. Коронарный кровоток, как и кровоток в других органах, зависит от соответствия метаболических потребностей тканей органа от реальных уровней и вариативности поступления к органу с кровью питательных веществ, кислорода и удаления конечных продуктов метаболизма. Любые малейшие отклонения от этого соответствия ведут к изменению кровотока. Наиболее значимым воздействием на коронарный кровоток является недостаток поступления к миокарду кислорода. В покое уровень кровотока в тканях сердца составляет ~0,8 ÷ 0,9 мл · г / мин. (~4% общей объёмной скорости кровотока сердца). При этом ткани сердца потребляют от 70 до 75% кислорода из протекающей через них крови. При интенсивной мышечной работе коронарный кровоток может возрастать в четыре-пять раз и примерно во столько же раз увеличивается потребление кислорода тканями сердца. В оттекающей от тканей сердца венозной крови коронарного синуса в норме содержание кислорода ниже, чем в крови, оттекающей от тканей любого другого отдела системы кровообращения. Указанные границы значений уровня потребления кислорода для состояния покоя являются максимальными. Увеличение потребления кислорода миокардом может произойти только при возрастании коронарного кровотока. В норме реальные потребности в кислороде и его актуальное использование почти полностью соответствуют друг другу. Это является результатом эффективного регулирования переменных кровотока. Уровень кислорода, содержащегося в оттекающей венозной крови коронарного синуса, практически не меняется, несмотря на изменения потребления кислорода тканями сердца. Средствами регулировании потока крови по коронарным артериолам являются многие метаболиты. Колебания коронарного кровотока во время сердечного цикла. Коронарный кровоток периодически изменяется в соответствии со стадиями сердечного цикла. Колебания кровотока обусловлены двумя фаторами: изменениями напряжения стенки сердца и пульсирующими изменениями давления в аорте. При напряжении сокращающегося и расслабляющегося сердца сдавливаются сосуды внутреннего и среднего слоев миокарда. Соответственно изменению диаметра потока крови изменяется сопротивление кровотоку. В начале систолы (систолическое сжатие) кровоток в левой коронарной артерии полностью прекращается. В это время, другая независимая регулируемая переменная коронарного кровотока - системное артериальное давление, т.е. перфузионное давление в коронарной артерии, является самым высоким. В диастоле, когда напряжение стенки миокарда снижается, кровоток в левой коронарной артерии достигает высокого значения. Систолическое сжатие оказывает гораздо меньшее влияние на кровоток в миокарде правого желудочка. Это связано с тем, что напряжение стенки сердца, снабжаемой правой коронарной артерией ниже и максимальное систолическое давление крови в правом желудочке существенно ниже, чем в левом. Соответственно, силы систолического сжатия стенки правого желудочка ниже, чем сжатие стенки левого желудочка. Поэтому кровоток в правой коронарной артерии зависит в основном от давления крови в аорте.
         В результате увеличения напряжения стенок сердца отток крови из коронарного синуса во время систолы резко возрастает. Во время диастолы отток крови из коронарного синуса уменьшается.
         Силы систолического сжатия коронарных сосудов в эндокардиальных (внутренних) слоях стенки левого желудочка, выше чем в эпикардиальных слоях. Соответственно, кровоток в эндокардиальных слоях левого желудочка во время систолы снижается в большей степени, чем в эпикардиальных слоях. В норме эндокардиальная зона миокарда за счет усиления кровотока во время диастолы может компенсировать уменьшение кровотока во время систолы. При патологии коронарных сосудов и при их стенозе эта компенсация может оказаться недостаточной. Поэтому инфаркты миокарда возникают чаще всего в эндокардиальных слоях левого желудочка.
         Регуляция коронарного кровотока.
         Даже в состоянии покоя сердце поглощает максимум кислорода из притекающей крови (~70 ÷ 75%), то есть намного больше, чем любой другой орган. В связи с этим, при повышении нагрузки на сердце его возрастающая потребность в кислороде не может быть покрыта за счет увеличения поглощения из крови кислорода. Поэтому повышенная потребность сердца в кислороде удовлетворяется главным образом за счет увеличения коронарного кровотока. Увеличение кровотока происходит при снижении гемадинамического сопротивления за счет расширения коронарных артериол. Наиболее мощным стимулом для расширения коронарных сосудов служит недостаток кислорода. Расширение артериол происходит уже при снижении концентрации кислорода в крови на 5%.
         К метаболитам, вызывающим расширение артериол относится аденозин. Это вещество играет важную роль в распаде макроэргических фосфорных соединений, и повышает внеклеточную концентрацию ионов калия. Средствами регулирования кровотока могут быть метаболиты эндотелиального происхождения (оксид азота, простациклин, эдотелин-1) и вещества, поступающие с системным кровотоком (норадреналин, серотонин, ацетилхолин, гистамин и др.).
         В регулировании коронарного кровотока существенное значение имеет миогенный механизм регулирования кровотока в артериолах.
         Прямое действие вегетативных нервов на коронарные сосуды трудно оценить, так как эти нервы одновременно влияют и на другие параметры деятельности сердца. Однако в недавних исследованиях были получены факты, свидетельствующие о прямом сосудосуживающем действии симпатических нервов и сосудорасширяющем парасимпатических нервов. Все эти факторы в совокупности образуют регуляторную систему, обладающую высокой надежностью. Недостаток того или другого компонента сильно отражается на регуляции. Критерии достаточности коронарного кровообращения. Резервы кровоснабжения сердца. Кровоснабжение сердца является достаточным в том случае, если поступление кислорода соответствует потребности в нем. Соотношение этих двух параметров служит критерием достаточности коронарного кровоснабжения. Его значения ниже 1,2 указывают на критическое уменьшение оксигенации сердца (например, при коронарной недостаточности). Для оценки состояния коронарного кровообращения важен не только этот показатель достаточности коронарного кровообращения, но также резервы кровоснабжения сердца, критерием которых служит разница между максимально возможной доставкой O2 и реальным потреблением кислорода в покое, деленная на это реальное потребление. При полноценной способности к адаптации резервы коронарного кровоснабжения в 4–5 раз выше, чем количество, потребляемое в состоянии покоя. Нервная регуляция коронарного кровотока. В условиях покоя сердечный кровоток равен примерно 0,8–0,9 мл •г –1 •мин –1, что для сердца массой примерно 300 г составляет около 250 мл/мин, или 4% общего сердечного выброса (табл. 20.5). При максимальной нагрузке коронарный кровоток может возрастать в четыре–пять раз, т.е. до 1250 мл/мин (табл. 20.6). На скорость коронарного кровотока влияют давление в аорте, частота сокращений сердца, вегетативные нервы, но наибольший эффект оказывают метаболические факторы. Регионарное кровообращение Каждые орган и ткань: мозг, сердце, легкие, печень, кожа, мышцы — обладают индивидуальными физиологическими особенностями кровообращения. Непрерывность движения крови в организме человека обеспечивается как системой последовательно соединенных сосудов, осуществляющих системную гемадинамику, так и системой параллельно подключенных к аорте и полым венам сосудистых русел, представленных сосудами различных органов и обеспечивающих регионарную гемадинамику. Хотя в каждом отдельно взятом органе {регионе) кругооборот крови в процессе ее движения не совершается, для обозначения гемадинамики в органах употребляется термин «регионарное кровообращение». Главное назначение кровообращения, в обеспечении обмена газами, веществами и продуктами их метаболизма, а также тепловой энергией между кровью и клетками тканей, реализуется на уровне сосудистой системы органов. Именно здесь осуществляется непосредственное соприкосновение обменных сосудов с тканевыми элементами, а структурные особенности строения стенки кровеносных капилляров и низкая линейная скорость кровотока в них создают оптимальные условия для полноценного осуществления обменно-транспортной функции кровообращения. Кроме того, процессы непрерывного приспособления организма к постоянно изменяющимся условиям внешней и внутренней среды вовлекают в активную деятельность различные регионы и группы органов. Это требует четкой координации и высокой надежности в адекватном перераспределении крови между работающими органами. Наконец, сосудистые сети органов, выполняя в общей схеме устройства сердечно-сосудистой системы роль параллельно включенных проводников, в значительной мере определяют величину общего периферического сопротивления сосудов и тем самым влияют на показатели системной гемадинамики. В то время как кровообращение в мышцах и большинстве внутренних органов определяется общими принципами и закономерностями, описанными выше, кровообращение в ряде регионов требует специального рассмотрения. 390 7.2.4.2. Венечное кровообращение Поперечнополосатая мускулатура сердца в отличие от скелетной характеризуется высоким потреблением энергии аэробного происхождения, что обусловливает значительную потребность миокарда в интенсивном кровоснабжении. Доставка артериальной крови в миокард осуществляется венечными (коронарными) артериями, которые, разветвляясь и широко анастомозирум во всех слоях и отделах сердца, образуют густую сеть капилляров и практически каждое мышечное волокно снабжено собственным обменным сосудом. Венозный отток от миокарда осуществляется через птрокий венечный (коронарный) синус, открывающийся в полость правого предсердия. Прекращение кровотока по коронарным артериям при их закупорке нли значительном спазме приводит к стойкому снижению кровоснабжения сердечной мышцы и к развитию инфаркта миокарда, что сопровождается нарушением нагнетательной функции сердца и может привести к смерти. Поскольку в системе коронарного русла достаточно хорошо представлен модульный принцип организации, аналогичные изменения кровотока в пределах отдельных сосудистых модулей могут проявиться в виде микроинфарктов, осложняющихся нарушением проводимости и сократимости сердечной мышцы. В состоянии функционального покоя у взрослого человека коронарный кровоток составляет 60—70 мл/100 г/мин. От общего сердечного выброса кровоснабжение миокарда составляет 4—5%, т.е. в среднем 200—250 мл/мин. В условиях интенсивной физической 392 работы, когда происходит активация сердечной деятельности, объёмная скорость кровотока в сердечной мышце возрастает, достигая 350—400 мл/100 г/мин (функциональная гиперемия). Коронарный кровоток существенно изменяется в зависимости от периода сердечного цикла. В период систолы желудочков интенсивность коронарного кровотока (особенно в миокарде левого желудочка) снижается, а во время диастолы увеличивается. Thus the driving force for myocardial blood flow is the systemic arterial pressure, just as it is for other systemic organs. Most of the blood that flows through the myocardial tissue returns to the right atrium by way of a large cardiac vein called the coronary sinus. Local Metabolic Control As emphasized before, coronary blood flow is controlled primarily by local metabolic mechanisms and thus it responds rapidly and accurately to changes in myocardial oxygen consumption. In a resting individual, the myocardium extracts 70–75% of the oxygen in the blood that passes through it. Coronary sinus blood normally has a lower oxygen content than blood at any other place in the cardiovascular system. Myocardial oxygen extraction cannot increase significantly from its resting value. Consequently, increases in myocardial oxygen consumption must be accompanied by appropriate increases in coronary blood flow. The issue of which metabolic vasodilator factors play the dominant role in modulating the tone of coronary arterioles is unresolved at present. Many believe that adenosine, released from myocardial muscle cells in response to increased metabolic rate, may be an important local coronary metabolic vasodilator influence. Regardless of the specific details, myocardial oxygen consumption is the most important influence on coronary blood flow. Systolic Compression Large forces and/or pressures are generated within the myocardial tissue during cardiac muscle contraction. Such intramyocardial forces press on the outside of coronary vessels and cause them to collapse during systole. Because of this systolic compression and the associated collapse of coronary vessels, coronary vascular resistance is greatly increased during systole. The result, at least for much of the left ventricular myocardium, is that coronary flow is lower during systole than during diastole, even though systemic arterial pressure (ie, coronary perfusion pressure) is highest during systole. This is illustrated in the left coronary artery flow trace shown in Figure 7–6. Systolic compression has much less effect on flow through the right ventricular myocardium, as is evident from the right coronary artery flow trace in Figure 7–6. This is because the peak systolic intraventricular pressure is much lower for the right heart than for the left, and the systolic compressional forces in the right ventricular wall are correspondingly less than those in the left ventricular wall. Normally, the endocardial region of the myocardium can make up for the lack of flow during systole by a high flow in the diastolic interval. However, when coronary blood flow is limited—for example, by coronary disease and stenosis—the endocardial layers of the left ventricle are often the first regions of the heart to have difficulty maintaining a flow sufficient for their metabolic needs. Myocardial infarcts (areas of tissue killed by lack of blood flow) occur most frequently in the endocardial layers of the left ventricle. 5 Consider that the endocardial surface of the left ventricle is exposed to intraventricular pressure ( 120 mmHg during systole), while the epicardial surface is exposed only to intrathoracic pressure ( 0 mmHg). Neural Influences on Coronary Flow Coronary arterioles are densely innervated with sympathetic vasoconstrictor fibers, yet when the activity of the sympathetic nervous system increases, the coronary arterioles normally vasodilate rather than vasoconstrict. This is because an increase in sympathetic tone increases myocardial oxygen consumption by increasing heart rate and contractility. The increased local metabolic vasodilator influence apparently outweighs the concurrent vasoconstrictor influence due to an increase in the activity of sympathetic vasoconstrictor fibers that terminate on coronary arterioles. It has been experimentally demonstrated that a given increase in cardiac sympathetic nerve activity causes a greater increase in coronary blood flow after the direct vasoconstrictor influence of sympathetic nerves on coronary vessels has been eliminated with -receptor blocking agents. However, sympathetic vasoconstrictor nerves do not appear to influence coronary flow enough to affect the mechanical performance of normal hearts. Whether these coronary vasoconstrictor fibers might be functionally important in certain pathological situations is an open question. Рис. The physiologic rationale is based on the requirement for coronary blood flow to meet the energy requirements of the heart. The main parameters dictating cardiac oxygen consumption are heart rate (chronotropy), cardiac contractility (inotropy) and left ventricular (LV) wall stress. Whereas coronary perfusion at rest in humans represents about 200 mL/min, it can increase up to 1000 mL/min on maximal exercise. The difference between values at rest and maximal levels of coronary flow represents the coronary flow reserve. The mechanisms by which the coronary bed adapts blood flow to the cardiac workload represent one component of coronary autoregulation, that is, the recruitment of the coronary flow reserve to match coronary blood flow (O2 supply) to energy needs (O2 demand). This is accomplished via metabolic byproducts and adenosine, but it can also be modulated through an integrated regulation of substance release from the endothelium or from the myocardium itself, neural control, myocardial compressive forces and aortic perfusion pressure. First, the myogenic tone of the small-caliber coronary resistance vessels is controlled by endothelial factors (such as nitric oxide), metabolic products (such as adenosine), and by the autonomic nervous system (both cholinergic and - and -adrenergic receptors). Second, this chemical control of coronary flow is additional to the control exerted by physical forces (aortic perfusion pressure and intramyocardial compression forces). All of these components are discussed in the first part of this Ch.. Unique Characteristics of Coronary Blood Flow Phasic Flow In contrast to most other vascular beds, the myocardium is perfused mainly during diastole and shows a sharp decrease in perfusion during systole, which can be attributed to myocardial compression (Fig. 54–1). The myocardial compressive forces are largely responsible for this phasic nature of coronary perfusion throughout the cardiac cycle. Because of this, a measurement of mean coronary vascular resistance is less meaningful than a calculation of vascular resistance at end-diastole prior to atrial contraction when compressive forces are minimal. A fall of coronary perfusion at the onset of ventricular systole is caused by the squeezing forces of the contracting myocardium, when the intraventricular pressure that opposes coronary flow is roughly equal to the aortic perfusion pressure. Blood flow through large coronary arteries can even be transiently reversed during early systole. Reciprocally, ventricular relaxation during diastole is accompanied by a drop in intraventricular pressure that is far greater than the decrease in aortic pressure, which allows optimal coronary perfusion early in diastole. In addition, the position of the coronary ostia just above the aortic valve leaflets favors the perfusion of the coronary bed during diastole by a retrograde blood flow from the ascending aorta. As a consequence, systole contributes for less than 20 percent of the total coronary flow during a cardiac cycle at rest. However, the fraction of coronary blood flow during systole can rise, particularly when tachycardia compromises diastole, as during electrical pacing or exercise (Fig. 54–2). Figure 54–1. Transmural Flow In a normal heart, the blood flow to the subendocardium is approximately 125 percent of that in the subepicardium,1 that is, the subendocardial-to-subepicardial (endo-epi) ratio, one index of myocardial ischemia, is above unity in the normally perfused heart. However, this ratio drops dramatically in conditions of reduced perfusion pressure or during coronary artery constriction or occlusion, particularly in situations with well-developed coronary collaterals, which favor the subepicardium. In that condition, the maintained subepicardial perfusion is at the expense of the subendocardium, and represents the basis of subendocardial ischemia. A reversed subendocardial-to-subepicardial flow ratio is the hallmark of myocardial ischemia. Furthermore, a vasodilator, for example, isoproterenol, can increase blood flow to the subepicardium at the expense of the subendocardium, a condition referred to as coronary steal. Another condition in which the subendocardial coronary flow will decrease, but not to the same extent as in ischemia, is cardiac hypertrophy. Not only the coronary arteries use their mechanism of autoregulation because of the higher energy demand of the hypertrophied heart, but also the compressive forces become much larger, especially in the subendocardium.2 Extravascular Compression As mentioned above, the compressive forces are largely responsible for the phasic nature of coronary flow. The subendocardium is the region most vulnerable to changes in physical forces. Not only the subendocardium is submitted to the highest compressive force during systole, but the subendocardial flow also can be impaired during diastole in conditions of increased LV wall stress, such as during the advanced stage of cardiac hypertrophy or during heart failure. An important distinction must be made for coronary blood flow to the left ventricle and right ventricle because of compressive forces, which are lower in the right ventricle. Correspondingly, the fraction of systolic coronary blood flow is slightly higher to the right ventricle than to the left, whereas the total flow is less in the right ventricle as a consequence of the lower level of wall stress.3 Oxygen Extraction Coronary oxygen extraction is almost maximal at rest. Myocardial oxygen extraction at rest averages 75 to 80 percent, and therefore it may increase only by a slight margin, up to a maximum of 90 percent, upon maximal demand, such as heavy exercise. As a consequence, the main mechanism to bring more oxygen to the myocardium upon increased demand is to increase coronary blood flow. Control of Coronary Blood Flow by Myocardial Metabolic Demand The primary determinant of coronary blood flow is myocardial oxygen consumption (MO2),4 which is calculated as the product of coronary blood flow and the arteriovenous (A-V) O2 difference. As noted above, because A-V O2 difference cannot increase greatly, an increase in MO2 must be accompanied by an increase in coronary blood flow. Because cardiac work is tightly coupled to MO2, the fact that heart rate increases threefold and contractility fivefold, along with increases in LV wall motion during maximal exercise, results in a requirement for a fivefold increase in coronary blood flow observed in that condition. Consequently, the main determinants of MO2 are those factors primarily involved in regulating myocardial metabolic demand (heart rate and contractility) and LV wall stress. Heart Rate Heart rate is the principal mechanism by which cardiac output can be increased. In humans, heart rate increases by about threefold during intense exercise as a result of an adrenergic stimulation from both neural and circulating catecholamines.5 Heart rate is probably the factor that affects MO2 the most. Increasing the heart rate to the same level as reached during exercise in an experimental preparation will increase coronary blood flow by 50 percent.6 During intense exercise, heart rate accounts for approximately 30 percent of the increase in coronary flow (see Fig. 54–2). In the case of tachycardia, the oxygen demand increases not only strictly because of a higher heart rate, but also because it decreases the diastolic time of coronary perfusion. Clinically, the Framingham study has shown that individuals with higher heart rates at rest have a higher probability of cardiovascular mortality. Myocardial Contractility The increase in contractile state of the myocardium is critical for adjustments to exercise and response to sympathetic stimulation. For example, during intense exercise, LV dP/dt (rate of change of LV systolic pressure), an index of contractility, can also increase four- to fivefold, which permits stroke volume to increase despite increases in heart rate. This increase in myocardial contractility also increases MO2, which requires a commensurate increase in coronary blood flow. Lv Wall Stress LV systolic wall stress correlates directly with MO2. The formula for LV wall stress demonstrates that this critical determinant of MO2 is directly proportional to LV pressure and LV diameter or volume, and inversely proportional to LV wall thickness. Accordingly, with LV hypertrophy caused by pressure overload, which increases LV systolic pressure, the wall stress is normalized by the LV hypertrophy, that is, increased wall thickness. For that reason, baseline myocardial blood flow per gram of tissue remains essentially at normal levels, 1.0 mL/min per g, in LV hypertrophy. For the same level of ventricular hypertrophy, that is, where wall thickness is reduced by LV dilation, MO2 will increase much more than in a condition of normalized wall stress.7 In patients with systemic hypertension, for example, MO2 increases, compared to normotensive subjects, before wall stress is normalized. This increased oxygen requirement implies a recruitment of the coronary flow reserve already at rest. The Adenosine Hypothesis As developed by Berne et al., the most potent stimulus that suppresses the myogenic tone and therefore dilates the coronary vasculature is adenosine.8 The ratio between the coronary blood flow after administration of adenosine and the flow at rest represents the coronary blood flow reserve. In normal individuals, this ratio is between 4 and 5, the flow ranging from 200 mL/min at rest to 1000 mL/min at maximal exercise in well-trained athletes. In cardiac myocytes in vivo, adenosine is formed when the rate of adenosine triphosphate (ATP) degradation exceeds the rate of ATP regeneration. This biochemical mechanism is extremely sensitive because the concentration of adenosine in cardiac tissue is about 1000-fold lower than the concentration of ATP. Therefore, even a minute imbalance between ATP degradation and synthesis will result in an increase of adenosine that will be qualitatively, if not quantitatively, very significant. Adenosine production is triggered by hypoxia, either in conditions of ischemia or when oxygen demand increases, such as during exercise. The "adenosine hypothesis" stipulates that the adenosine released from the myocytes binds to A2 receptors on the smooth muscle cells of the coronary bed, which stimulates vasodilation through production of cyclic adenosine monophosphate.9 The resulting decrease in vascular resistance increases the coronary flow, which restores the balance between oxygen demand and supply. The same adenosine that is formed under these conditions will bind A1 receptors on the cardiac myocytes to decrease inotropy, and will trigger the mechanisms of preconditioning.10 Inhibition of adenosine receptors does not abolish the coronary autoregulation in absence of ischemia, suggesting that, in physiologic conditions, the endothelial control of coronary flow predominates, whereas in ischemic conditions, the metabolic control takes over through the production of adenosine. However, nitric oxide production increases in conditions of ischemia11 and may therefore be complementary to the effects of adenosine. Coronary Autoregulation Coronary autoregulation represents a mechanism by which the coronary artery can maintain a constant flow that matches the metabolic demand independently of the perfusion pressure. The coronary flow can increase up to fivefold between the basal state and the maximal perfusion, which represents the coronary flow reserve. Therefore, coronary autoregulation represents a mechanism by which the coronary flow reserve can be recruited to maintain the desired perfusion level. It is important to note that there are two distinct definitions of coronary autoregulation: the one controlled by the perfusion pressure and the one that is an adaptation of coronary resistance to the metabolic demand. These two components, or definitions of coronary autoregulation, are discussed next. Autoregulation by Pressure Coronary autoregulation is a mechanism intrinsic to myogenic tone in the coronary vessels by which the coronary artery maintains a constant blood flow in the face of variations in perfusion pressure.12 The vascular smooth muscle cells making the tunica media of the coronary arteriole are rich in stretch-operated ion channels activated by shear stress and which stimulate cell contraction. The high myogenic tone that results from the contractile activity of the smooth muscle cells translates in a high resistance to flow. This process allows keeping the flow constant when the perfusion pressure varies between 40 and 140 mmHg.12 These values were first established in experimental models, and subsequently confirmed in humans. The result is that, in cases of severe coronary artery stenosis or in shock, when the vascular resistance has fallen to its maximal capacity, the flow becomes directly proportional to the perfusion pressure. Coronary Autoregulation by Metabolic Demand The main cause of coronary stenosis and obstruction of a coronary artery is atherosclerosis. The single most important parameter of a coronary stenosis is the extent of reduction of the internal luminal diameter because, according to the laws of hydrodynamics, the drop in perfusion pressure across the stenosis is inversely proportional to the fourth power of the minimal luminal diameter. In other words, even a slight decrease in diameter will result in a major loss of pressure. As an example, it has been calculated that an atherosclerotic plaque progressing from an 80 percent to a 90 percent obstruction of the vessel diameter would result in a threefold increase in coronary resistance. The effect of coronary obstruction on blood flow therefore depends largely on the extent of the stenosis. For example, although minor obstructions do not have hemodynamic repercussions, a coronary stenosis obstructing at least 40 percent of the vessel diameter results in a drop of perfusion pressure, which requires a matching decrease of coronary resistance to maintain the normal flow at rest.13 The decreased resistance is mediated mainly by adenosine release from the myocardium supplied by the obstructed artery. This process requires recruiting the coronary blood flow reserve already at rest, and therefore limits the capacity to exercise. If the obstruction progresses, the coronary blood flow reserve will progressively decrease. When the coronary stenosis becomes so severe that it occludes at least 80 percent of the vessel diameter, the coronary reserve will be totally recruited at rest, and, at that point, any further obstruction will result in a limitation of blood flow already at rest. This concept is further developed below in the section on Coronary Blood Flow Reserve. Control of Coronary Blood Flow by Non-Metabolic Factors Control of Flow by the Endothelium The endothelium both produces vasoactive substances (nitric oxide, prostacyclin, endothelin-1) and responds to circulating factors (e.g., epinephrine, serotonin). The overall function of the endothelium is to increase the flow through the arteries, if we exclude the tonic vasoconstriction induced by endothelin-1. Consequently, endothelial dysfunction resulting from endothelial damage that accompanies most risk factors (in particular atherosclerosis) will result in increased vascular resistance and impaired flow. The most potent vasodilator released by the endothelium is nitric oxide (NO), which is produced by the stimulation of the cholinergic cascade, and which immediately diffuses as a gas from the endothelium to the surrounding vascular smooth muscle cells where it induces a cyclic guanosine monophosphate-dependent muscular relaxation.14 Paradoxically, acetylcholine provokes vasoconstriction in isolated coronary arteries in vitro. These apparently contradictory results can be reconciled in light of the seminal observation by Furchgott and Zawadzki15 that acetylcholine induces vasoconstriction, rather than vasodilation, in isolated aortic rings denuded of endothelial cells, but in vessels with endothelium intact, acetylcholine induces prominent vasodilation. NO is the main mediator of the vasodilatory effect of acetylcholine (which is released physiologically through the parasympathetic neural stimulation) but its production is also increased by stimulation of the -adrenergic receptors. In addition to NO, another potent vasodilator released by the endothelium is prostacyclin, which is involved in the normal vasodilation at rest and in the adaptation to increased shear stress. Blocking prostacyclin synthesis with cyclooxygenase inhibitors (e.g., aspirin, indomethacin) can reduce the coronary blood flow at rest in patients. Control by the Autonomic Nervous System Parasympathetic (Muscarinic) Receptor Control The release of NO by stimulation of endothelial cells by acetylcholine (the parasympathetic neurotransmitter) remains one of the most important mechanisms of vasodilation throughout the vasculature, which is reflected by the use of nitrates to decrease the preload in patients with coronary artery disease. Vagal stimulation has a vasodilatory effect in the coronary bed, thereby increasing coronary blood flow. In addition to a direct effect on the coronary bed, the parasympathetic tone reduces heart rate, arterial pressure and inotropy, which altogether reduce the oxygen demand of the myocardium.16 However, when these parameters are kept constant in experimental preparations where both heart rate and perfusion pressure can be controlled, vagal stimulation still increases coronary flow. This effect is reproduced by administration of acetylcholine and is blocked by atropine. It is now well established that endothelial dysfunction and reduced NO production, which often accompanies atherosclerosis, increases the frequency of coronary vasospasm. Even if the coronary obstruction resulting from the atherosclerotic plaque is minor, the risk of vasospasm in that condition may be sufficient to induce ischemia and even myocardial infarction. -Adrenergic Receptor Control The stimulation of cardiac sympathetic nerves markedly increases coronary blood flow, as a consequence of the increased metabolic demand induced by the increase in workload (heart rate, contractility, LV wall stress) resulting from catecholamine release. This increased metabolic demand is blocked by pretreatment with a -adrenergic antagonist, which unmasks a vasoconstrictive effect.17 This vasoconstriction, in turn, can be blocked by an -adrenergic antagonist.18 These results indicate that sympathetic stimulation in vivo increases heart rate and inotropy mainly through the -adrenergic receptors, whereas stimulation of the receptors induces a vasoconstrictive response. Similarly, direct stimulation of 1-adrenergic receptors with pharmacologic agonists can provoke vasoconstriction in coronary vessels. An important consequence of the -adrenergic–mediated vasoconstriction in a context of increased workload is an increase in oxygen extraction,19 which might be particularly relevant during exercise. The vasoconstriction resulting from -adrenergic receptor stimulation is increased in patients with coronary artery disease,20 probably reflecting the partial use of the coronary flow reserve in that situation. The -adrenergic tone of the coronary arteries can be activated by stimulation of reflex pathways, such as carotid baroreceptors upon carotid sinus hypotension, as well as by direct sympathomimetic stimulation. Similarly, stimulation of the carotid chemoreceptor, pulmonary inflation reflex, and arterial baroreceptor hypertension induce coronary vasodilation that results from a withdrawal of the -adrenergic tone, potentially with a smaller vagal component contributing to the vasodilation (Fig. 54–3). Circ Res 1975;37:664–673. -Adrenergic Receptor Control Stimulation of the -adrenergic receptors achieves coronary vasodilation through two complementary mechanisms. Binding of the receptors in the coronary vessel itself will stimulate a Gs protein-mediated relaxation of the vascular smooth muscle cells.21 In addition, binding of the receptors on the cardiac cells will stimulate a Gs protein-mediated increase in contractility, which, together with the increase in heart rate, will increase the metabolic demand and activate the recruitment of the coronary flow reserve through adenosine release. Whereas the 2 receptors are mainly responsible for the direct vasodilatory mechanism, the contribution of the 1 receptor predominates for the metabolic effect resulting from increased workload.22 As a consequence, treatment of patients with blockers will reduce coronary flow and may exacerbate coronary vasospasm. However, the negative inotropy and reduction of heart rate following administration of blockers markedly reduce the risk of exercise-induced angina, do provide protection following myocardial infarction, and, accordingly, are used widely in patients with ischemic heart disease as well as in hypertension. Control of Coronary Blood Flow in Large Arteries Under normal circumstances, large coronary arteries participate very little in the regulation of coronary resistance because the chemical control of coronary blood flow is mainly performed at the level of small-caliber resistance vessels. However, the large coronary arteries play a major role in the regulation of coronary flow in pathologic settings, especially in conditions of coronary vasospasm and obstruction by an atherosclerotic plaque when blood flow becomes largely dependent on perfusion pressure, and by shear stress (flow-dependent regulation of flow). The physical components of coronary flow include the coronary perfusion pressure, flow-dependent regulation, as well as control by neurohumoral mechanisms. In contrast to the phasic measurement of coronary blood flow, the phasic measurement of large coronary artery diameter parallels that of aortic pressure (see Fig. 54–1). Flow-Dependent Dilation Because NO production is also increased by shear stress, it represents an excellent mediator of autoregulation in conditions of increased cardiac output, such as during exercise.23 This mechanism represents the molecular basis of flow-dependent coronary vasodilation. The increased shear stress in conditions of increased flow, which is mainly determined by the viscosity of the blood and its velocity, represents a stimulus for NO production that is followed by vasodilation and subsequent decrease in shear stress. One of the first investigations was by Hintze and Vatner, who observed that following a temporary coronary artery occlusion during the reactive hyperemia phase, the large coronary arteries dilated (termed "reactive dilation"), which was entirely dependent upon the increase in coronary blood flow.24

    Предпосылка:
    Эффективность развития любой отрасли знаний определяется степенью соответствия методологии познания - познаваемой сущности.
    Реальность:
    Живые структуры от биохимического и субклеточного уровня, до целого организма являются вероятностными структурами. Функции вероятностных структур являются вероятностными функциями.
    Необходимое условие:
    Эффективное исследование вероятностных структур и функций должно основываться на вероятностной методологии (Трифонов Е.В., 1978,..., ..., 2015, …).
    Критерий: Степень развития морфологии, физиологии, психологии человека и медицины, объём индивидуальных и социальных знаний в этих областях определяется степенью использования вероятностной методологии.
    Актуальные знания: В соответствии с предпосылкой, реальностью, необходимым условием и критерием... ...

    Любые реальности, как физические, так и психические, являются по своей сущности вероятностными.  Формулирование этого фундаментального положения – одно из главных достижений науки 20-го века.  Инструментом эффективного познания вероятностных сущностей и явлений служит вероятностная методология (Трифонов Е.В., 1978,..., ..., 2014, …).  Использование вероятностной методологии позволило открыть и сформулировать важнейший для психофизиологии принцип: генеральной стратегией управления всеми психофизическими структурами и функциями является прогнозирование (Трифонов Е.В., 1978,..., ..., 2012, …).  Непризнание этих фактов по незнанию – заблуждение и признак научной некомпетентности.  Сознательное отвержение или замалчивание этих фактов – признак недобросовестности и откровенная ложь.
